What happens during my first visit?

Download and fill out our patient forms so you can have them ready. You and your therapist will discuss your medical history and how your pain is affecting your life. An objective evaluation will include:

 • History and special tests

 • Range of motion (moving the joints)

 • Muscle and neurological testing

 • Joint and soft tissue assessment

What do I need to bring with me?

Make sure you bring your physical therapy referral (provided to you by your doctor) and your payment information. If your therapy is covered, bring the appropriate payment information:

 • Insurance card

 • Workers’ Comp claim number

 • Case manager’s contact information

 • Auto insurance or attorney lien info

How should I dress and how long will my treatment take?

Wear loose-fitting / comfortable clothing.  If the problem is your knee, wear or bring shorts. If you have a shoulder problem, a tank top is probably a good idea. For low back problems, wear a loose fitting shirt and pants so we can evaluate the area easily.

 Treatment sessions typically last 30-60 min.
